Hole-Watch
Ok, We need to get into the manhole and perform some work let's get Anybody to stand by as our confined space hole watch, Anybody asked the leadman “yes grab anybody “so Anybody was asked to be the hole watch
• Later that day the leadman and foreman were overcome with carbon monoxide due to Anybody walking away from hole to get something to eat and a vehicle parked next to the man-hole.
Spotter
• Let’s go yelled the foreman get the crane set up, Anybody go spot for the crane. Anybody ran eagerly over to be a spotter.
• Few minutes later the crane boom came into contact with overhead lines, Anybody was distracted watching the other crew members working and not the crane movement.
Firewatch
• Maintenance was called to repair a broken bracket in the warehouse, Give me Anybody to stand fire watch so safety doesn’t get onto us. • While repairing the bracket, sparks from a grinder flew onto some flammable containers igniting a major fire. Anybody was on social media and not paying attention
In all the above scenarios Anybody should not have been chosen,
